Kern Legacy Health Plans With four plans to choose from, there’s a plan with the benefits and doctors for your needs Kern Legacy SHARE SELECT “Share in good health. Share in the savings.” Share Select uses Kern Legacy’s original, exclusive network design, with Kern Medical as the premier hospital, along with a solid selec- tion of other community providers, Select Network. This plan also adds a health savings account (HSA)—a compatible, high-deductible design to generate even more employee savings. For many services (except preventive care and certain medications), the member pays the entire contracted rate before the deductible is met. However, these higher out-of-pocket costs allow premiums to be low or at no cost. If you’re in good health and make good health care choices, you can save significantly. The money you save on premiums can also be contributed to a tax-free and fully portable HSA that you can use for any unexpected medical expenses. Kern Legacy NETWORK PLUS “The benefits and services you love. Plus.” Network Plus is a uniquely designed, two-tiered EPO (Exclusive Pro- vider Organization) plan using our custom-built network. Members have access to all the same providers as the Kern Legacy Share Select plan on the first tier, including Kern Medical as the preferred hospital and, in addition, an expanded selection for most specialties with ser- vices at low to no out-of-pocket costs. The second tier gives members even more choices with the “Plus” Network. With this tier, members can access another list of providers, including Adventist Health hospi- tal. Members share 20% of the cost, after meeting a small deductible, for services when using the Plus tier. Network Plus is perfect for anyone looking for a quality, affordable health plan with plenty of options. Kern Legacy MAX CHOICE “Maximum options. Maximum flexibility.” Kern Legacy Max Choice takes the idea of options even further. Similar to the “Plus” benefit of Network Plus, Kern Legacy Max Choice features an affordable 20% co-insurance and low deductible, but it adds the same Anthem Blue Cross network of providers as the County’s original benefit plan, Kern Legacy Classic Choice (formerly the “Point of Service Plan”). Max Choice covers the majority of local physicians, facilities and hospi- tals, giving you plenty of flexibility for your care. Anthem Blue Cross offers a nationwide network. Up to 90% of providers in California are partici- pating Anthem providers. If you travel often or have dependents who live outside of Kern County, Kern Legacy Max Choice will have you covered. Kern Legacy CLASSIC CHOICE “The original County health plan. Truly classic.” Kern Legacy Classic Choice is the traditional County self-insured benefit plan that started it all. Classic Choice features preferred co- pays at Kern Medical, no deductible for in-network services and an out-of-network benefit. Classic Choice uses the same Anthem Blue Cross network of providers as Kern Legacy Max Choice, covering the majority of physicians and hospitals locally, statewide and nation- ally. Since most services are covered at almost 100% with only small co-pays, premiums are highest on this plan. Classic Choice offers a large network and more traditional health plan design, with the cost of services primarily paid through premiums rather than individual members’ cost-sharing.
